Request for information on Personal Data

In order to protect your privacy and that of other data subjects, we need to confirm that you are the actual data subject of the data queries, as the law ensures that only you or your legal representative can make requests regarding the processing of your data. Therefore, if you are a registered user on our website, we need you to login first and then proceed to fill out and submit the form.

If you are not a registered user on our website, you can proceed with filling out the form, but the submission must be accompanied by an image (photo or scan) of your photo ID, your CPF and a current proof of residence (last 30 days) in your name.
